What steps will reproduce the problem?
1. Following the instructions, I get the following:

Creating USB Flash Disk...
  umount_disk settling delay (10 seconds)
  partition the target disk
  umount_disk settling delay (10 seconds)
  install recovery onto target disk
Unable to mount image: hdiutil: attach failed - Resource busy
  install_recovery failed
Failed! (0:00:47)

Having identical problem using both a Kingston 2GB stick and a Gigaware 4GB 
stick. This is on Mac OS 10.5.8 
with the DMG released 2/9/10: "2Z694-6013-013.dmg"

The fix offered by sdavilla in Issue 292 worked for me: restart OSX and 
relaunch ATV USB Creator, follow 
instructions as before.
