I think I'm missing a step

What steps will reproduce the problem?
1. I download the patchstick image and restore it to my usb drive but it 
restores only as one file (which I've called patchstick.img).  Boot the ATV 
with the USB drive and nothing happens.

Is the restored USB supposed to have more in it than just the .img file?

What is the expected output? What do you see instead?
Not exactly sure, but right now I see nothing

What version of the product are you using? On what operating system?
atv-win-3.0
Windows Vista

Please provide any additional information below.

You restore a disk image to the USB which has all the partitions and files 
inside 
it. Put it in the aTV and fire it up - should load. If nothing happens try 
holding 
Menu and - for 6 seconds.

If you mean you copied the patchstick.img onto the usb drive then no, this is 
wrong. 
You are supposed to restore it when asked to with USB Image Tool
