Still failing to complete the patch #579

What steps will reproduce the problem?
1. follow all provided instructions. I prepared several USB on win7 and MAC-OSX.
2. ATV identified the boot USB and failed to load patchstick. I tried several 
times.
3. I tried also with menu + '-' keys pressed while booting.

What is the expected output? What do you see instead?
The boot process ended with request to login..... 
rebooting ATV simply showed again the standard ATV menu without the additional 
XBMC menu item

What version of the product are you using? On what operating system?
ATV 3.0.2
2Z694-6013-013.dmg (done via USB IT)

Please provide any additional information below.
This is the first time I try to patch my ATV. 
I can't telnet or SSH. I get connection refused message. I tried via wireless 
and wired connection.

I tried to follow previous discussions, but seems like all other managed to 
login into ATV, where I can't.

Still failing.
I have tried about 9-10 different kind and models. 
All ending with the same result of booting from the USB but later for some 
reasons the USB is not mounted. ending the process with telnetd and waiting for 
local login.

When I telnet to ATV at this stage, /dev/sd* holds only the 5 sda, sda1... 
devices. no sdb* devices.

is it a problem of the loader that doesn't create the right device?

I tried more than a dozen different USB drives.
Also tried to use a 2.2 dmg image.
nothing helped.
Is there any way that I can copy all payload from a remote machine while logged 
in the ATV?
and than, after it is in ATV, to run it from the ATV itself instead of the USB 
drive....

rcp? ftp? scp?

Also, will it help if I reset my ATV to factory defaults? 
it is a 1.5-2 year old box...

Must be careful to follow directions on formatting the USB. Use disk utility, 
partition (create 1 partition). Then format w/ FAT (creating the single 
partition will force the FAT32). If this is not done the USB will not work. 
That happened to me the first time.
