Header with graphic only content is not repeated when Table without FieldsMetadata has more than one page #232

What steps will reproduce the problem?
1. Extend DocxTableWithoutFieldsMetadataWithVelocity.docx example with a header 
that contains only graphics
2. Provide dummy data to get more than one page

What is the expected output? What do you see instead?
- Header should be on all pages
- Header is only shown on the first page 

What version of the product are you using? On what operating system?
1.0.0 WIN7

Please provide any additional information below.
-> Workaround: Include a dummy letter with the background color

Please provides a sample (docx+java) which causes the problem.
