[mockup] Evidence Type #185

  1. change study type to evidence type
  2. modify fields in evidence type 2.1 add selected method 2.2 add question list(yes/no) 2.3 when new evidence type is available based on users' selection, show the list of evidence type 2.4 user can choose evidence type from list
