Refactor and Updates

This PR updates the VSCode extension to download and launch the Debug proxy when time travel debugging.

• Downloads the latest version of the debug proxy from the public S3 bucket
• Launches the debug proxy automatically when starting a debug session if it's not already running
• Reads prov DB connection info from VSCode configuration for both proxy launch and workflow ID retrieval
• Retrieves prov DB password from user directly and stores in vscode secret storage
• Adds commands to launch/shutdown proxy manually and to clear the stored prov DB password from secret storage
• Updated logging infrastructure to use Winston like we use in DBOS SDK, but with a custom VSCode output window transport
• redirects debug proxy console logs to a 2nd VSCode output window

The big still-missing feature for this extension is the WF ID picker. A list of WF ID UUIDs w/o additional context is pretty useless. This experience will be addressed in a later PR and/or with help from Alex and the user-facing grafana dashboard
