remove phonePrefix, keep areaCode

Props phonePrefix and areaCode are duplicate.

phonePrefix is used in under 100 maps, areaCode in over 250.
phonePrefix is defined as xsd:nonNegativeInteger (worse), areaCode is xsd:string (better)

I changed most of these, but there are two or three left where there are more than one kind of code, which need a closer look.
