add link to issues, incorporate issues into agenda #15

This PR includes a 1) manually-crated table of issues (labelled with proposed session) into the agenda and 2) add more pointers to the agenda and the issues in the README file. I'm hoping for better information flow.

relates to https://github.com/dcppc/2018-june-workshop/pull/13 which created an automatically generated list of issues in a new page called issue-table

Thanks! I tried like 15 times and couldn't quite get it right, so I just deleted the shield. I'll re-add it
