New user repeatedly prompted for challenge questions and answers - invalid challenge set configuration

What steps will reproduce the problem?
1. Create new user
2. Enter challenge questions and answers
3. Confirm challenge questions and answers
4. User is prompted to enter challenge questions and answers again.

What is the expected output? Main Menu What do you see instead? Further prompts 
to complete challenge questions and answers.

What version of PWM are you using? Daily Build from around 20 Septemmber 2014

What ldap directory and version are you using? Active Directory

The issue appears in the logs when a configuration is saved:

2014-09-25T03:55:29Z, WARN , config.ChallengeProfile, invalid challenge set 
configuration: too few challenges are required
2014-09-25T03:55:24Z, WARN , pwm.PwmApplication, configuration has been 
modified since last startup
