Refactor diagram

Use A4 landscape
Arrange elements more precise
Add template text for easier editing
Use layers for easier editing
Add more usage options to instructions

It looks very good for me and we need to work on our process to not let valuable PRs going stale :) I would merge it now but I don't have the rights. @NTCoding or @cakper can you please fix this (either enabling me for this and/or merging the PR)? Thanks @AlexZeitler for the patience :)

Hey @yellowbrickc,

You mentioned after the last PR that you wanted to review so I left this one for you.

You should have permissions to merge since you are a member of the ddd-crew. What options do you have available? I've not been able to find any settings that are at the user-level, only the team level.
