Factors with more levels defined than actually used as medication class disrupt plot.CMA0 #109

At least for medication class (maybe others as well?) if the variable is a factor with more levels than actually used (say, from a subsetting of a bigger vector) then plot.CMA0() -- maybe others as well -- fail to plot the medication classes
