Attachment content type is ignored #13

With the latest trunk, when I use the Doc.attach() function to upload an attachment, the content-type shows on the Couch server as "application/octet-stream" regardless of the value provided to the attach function.

Fixed it in the master .. could not test though, since I don't have an installed couchdb right now. Can u pls check and let me know your feedback.
