Dispute attachments fails to upload if the file is big #150

Right now, if you try to upload a file that is too big, it fails to upload.

Here we try to upload a 5mb PDF.

Acceptance criteria

[ ] Limit uploads to 5mb.
[ ] If the user tries to upload a file bigger than 5mb, show a message saying the file is too big.
- [ ] The message suggests ways to solve the problem (email the dispute admins?)
[ ] Implement timeout handling for uploads, if an upload gives you a timeout, we should ask the user to upload the file again.
[ ] Implement error reporting so that we are notified when anything similar happens.
