This PR tackles some issues with the main navigation (#53)

Removed the FormMenuActions component
Changed onUnmounted to onBeforeUnmount in AppLayout, Edit, Settings and Summary pages
Added a new NavigationSpacer component for spacing out navigation buttons better
Updated FactoryNavigation to use href instead of route-name so that we can link outside of forms as well (e.g., dashboard)
Created PublishToggle which is used by FactoryNavigation now instead of FormMenuActions was before
