Bugfix/lv claim #18

LiquidVault's claim hotfix:

added claimed boolean flag for a batch to indicate whether it was claimed or not
if the latest batch in the queue was claimed & the user will call claimLP once again, tx will revert
updated the tests & run them locally
checked on Kovan
