feat(grafana): Add deis-health dashboard #114

manual testing steps:

  1. cd grafana
  2. make build push upgrade
  3. goto grafana.domain.com use admin/admin to login
  4. From the drop down at the top left select Deis Health
  5. observe that you are seeing memory and cpu stats being graphed

@jchauncey I didn't manually test this yet. Is it just building & deploying the new image?

Tested as per above instructions. Looks good: screen shot 2016-06-27 at 12 42 09 pm
