This is a feature request and not a bug report :)  Would it be possible to
support internal cross references, perhaps in the manner of
http://fletcherpenney.net/MultiMarkdown_Syntax_Guide#automaticcross-references

This feature is supported as of r1086.

r1093 reverts changes in r1086.  Unfortunately, these changes caused
too much of a performance hit.  For the time being, anyway, references
to section headers must be explicit:  [My header](#my-header).
