On some platforms, warnings are repeated many times #72

I've seen an instance where on MacOS X the vaxis warning gets repeated every time the PPV statistic gets created/used. Need to debug this.

I can't reproduce, but with this change we explicitly enforce that warnings should only be issued once, and I've also made the verbose option work.
