Rename PackIndexTest to PackIndexTestCase #75

When trying to run tests via an ant runner using a pattern of **/*Test.java
the lone PackIndexTest (which is abstract) gets picked up. The other
abstract test cases are named *TestCase.java. Changing this one file would
make it easier to test with ant.
