Make "Topics", "Facility", "Creators" and "Persons/Organizations" two columns wide, akin to the "Contributors" field on Entity Detail page

Also move "Facility" before "Topics"; making display order: "Facility," "Topics," "Creators," then "Persons/Organizations"

Fixed in branch 206-entity-fields-display, merged in to develop and pulled to ddrstage.densho.org for review.

Changes are ready for viewing on ddrstage.densho.org. I couldn't get fontawesome to work so i just used the glyphicons from Bootstrap.
