Please provide the following information:

@rrubenstei002 why do we need a "justification"? unless we need a data item we shouldn't ask for it, right? if we need to collect this, should we give them options to choose from?

What will your team use GH for?

@rrubenstei002 also i thought Lisa had suggested adding a field for VA Business Point of contact (this is required given a vendor could be the admin right?)

That’s a great idea. Because this is a shorter form and we’re collecting less information Drew thought we should add a field that asked why someone was requesting access. We definitely need to flesh the fields out for final form.

Drew was confused about that field so I left it off for the MVP until we can get clarity on it.
