As a user, I want to know that I'm ineligible for appointment requests, so that I don't waste my time going through the request flow.

Acceptance Criteria

• [ ] Message that tells the user they're unable to request or schedule appointments online.
  • [ ] Message includes reason (not registered to VA facility)

To request an appointment online, you need to be:

Also, wondering if the wording of this ticket should be updated? I won't be providing any visual artifacts/wireframes -- just copy. Should I change the description or you?
