What happened?

Add the FE logic to include the CC providers address for Pending CC requests once backend finishes ticket: 8852

Acceptance Criteria

CC provider's address's are displayed under the Preferred community care provider fields

@rbontrager is this ticket closed via backend change? As in, did the BE include the address in a place that FE doesn't require any changes? If not we'll need some more specifics about where and what we need to edit.
