User accounts should be approved and created by a member of our team via the usual protocols, and they should not be automatically created within the system if a user with no account attempts to log into https://prod.cms.va.gov/ using their PIV card.

Steps for Implementation
Turn off User Provisioning and update user creation workflows.
Acceptance Criteria
[ ] Users who attempt to log into https://prod.cms.va.gov/ using a PIV card who do not have an existing account will not have one automatically created for them.
[ ] If this happens, the user should be re-directed back to instructions on how to contact the VA Drupal CMS Support team for an account
[ ] "Ghost" accounts are no longer added to the site.
