Revision Log showed Wrong Editor Making Update

Description of Defect

An Editor spotted an update made at 10:32pm indicating that her account had made the update, however she did not make this update. Her PIV card is secure and she submitted a ticket with the Help Desk to raise the issue.

Additional Context from Michelle Middaugh: additional context: we put up a banner alert related to the phone issues noted for the Veterans Crisis line on August 28, 2024 at 10:55 p.m. ET and it was applied to most VAMCs, including Coatesville

Michelle's thoughts

in the interim - the explanation could be that we have done it on their behalf
but ideally it should reflect the person who did it - or "Admin", possibly
given existing behavior, it would be tricky to track down the actual editor, if needed and any notifications would misfire

A couple other notes

This inaccurate information only showed up in the right-hand revision log.

Steps for Implementation

Investigate why Melanie was associated with the update that she did not do.
Look into Michelle's ideas as to why and confirm is it is correct or incorrect.

Acceptance Criteria

[ ] An explanation to why the wrong editor was associated with the update in the revision log.
[ ] A plan of action to address the defect.
