[CONSIDER] Improve Vet Center CAP form Access Point name UX #4891

Description

As a Vet Center editor (or creator!) of a CAP, i want a form that helps me to understand how the content will be displayed to Veterans, so that i can name my CAP appropriately.

Background

CAP names are a combination of the Vet Center name and an "Access Point name" (fka "Geographic identifier"), eg "Orlando Vet Center - Sanford". Only the Access Point name is contributed by Vet Center staff.

The design suggests a "Prefix" style approach that prepends the Access Point name field with the name of the Vet Center.

How might we accomplish this design?
