@sonntag-adam I signed in as Vet User 14 and when I arrived at the contact information page, it was already filled in. When I open the Phone & email section, the label titles for the two phone numbers are correct, but not when its collapsed like this.
@sonntag-adam One final change - the heading "Form 22-0994" needs to appear beneath the "Apply for VET TEC" heading. (See SA #1) It's currently on the same line.
DEPO has weighed in on whether the form name and number needs to appear beneath the heading and DEPO says its fine as is
