0994: Form Content Updates #16721

As a developer, I need to update the 22-0994 form so that it is consistent with DEPO recommendations and other Education Services forms.

Assumptions:

  1. This update does not require a feature or prod flag; story will be merged at the beginning of the day for staging review prior to afternoon production deployment.
  2. The 0994 is located here: https://staging.va.gov/education/about-gi-bill-benefits/how-to-use-benefits/vettec-high-tech-program/apply-for-vettec-form-22-0994/introduction
  3. The content updates are based on the DEPO form labels here: https://design.va.gov/patterns/form-labels

Acceptance Criteria

  1. The following chapter titles are sentence case: a. Applicant information b. Service history (renamed from "Military Service") c. Education history d. Work experience e. Program selection f. Personal information

  2. In chapter 6: a.

    Street address (line 2)

    is replaced with

    b.

    Street address (line 3)

    is replaced with

    Line 3

    c.

    Phone number

    is replaced with

    Home phone number

    d.

    Alternate Phone number

    is replaced with

    Mobile phone number

  3. On the submission confirmation page: a.

    Your claim has been received

    is replaced with

    We’ve received your application.

    b.

    Education Claim (Form 22-0994)

    is replaced with

    Education benefit application (Form 22-0994)

    c. The content in the "What happens after I apply?" section is:

    We usually decide on applications within 30 days.

    You’ll get a Certificate of Eligibility (COE) or decision letter in the mail. If we’ve approved your application, you can bring the COE to the VA certifying official at your school.

    d. The padding above "Education Benefit Claim (Form 22-0994)" on the confirmation submission screen is reduced. (Ex. the 10203 has the class vads-u-margin-top--0; per UX 23px is the appropriate amount of space). e. The "Need help?" section style matches DEPO patterns (see SA2). f. The confirmation page layout and styles match that as shown in SA#3

@sonntag-adam I signed in as Vet User 14 and when I arrived at the contact information page, it was already filled in. When I open the Phone & email section, the label titles for the two phone numbers are correct, but not when its collapsed like this. Screen Shot 2020-12-11 at 7 53 52 AM

@sonntag-adam One final change - the heading "Form 22-0994" needs to appear beneath the "Apply for VET TEC" heading. (See SA #1) It's currently on the same line. Screen Shot 2020-12-15 at 7 04 09 AM

DEPO has weighed in on whether the form name and number needs to appear beneath the heading and DEPO says its fine as is
