As a developer, I want to support POSTing to the requests endpoint so that the frontend can support submitting new appointment requests.

[ ] use staging console to retrieve json and convert the json into vcr cassettes for future playback
[ ] requests endpoint supports POST method with success using data from cassette
[ ] Tests, specs, and swagger exist

PR is merged and code validated in staging UI.
