Discovery efforts for agility in the Collaboration Cycle

Issue Description

Before implementing a solution to help improve agility in the Collaboration Cycle, we must first have a complete and thorough understanding of the current problems. We will be completing a "5 Whys" exercise to get to the root cause of each problem statement identified in the Initiative Outline. After completion of the "5 Whys" we will prioritize the identified problems. For the top priority problems, we will complete a rapid ideation/rapid prototyping session. The team will vote on which solutions to move forward with in order to make Collaboration Cycle more agile.

[x] Potential solutions have been validated for scalability and LOE
[x] Potential solutions address identified pain points
[ ] OCTO-DE has approved the proposed solutions

@shiragoodman in your acceptance criteria, please include something around OCTO-DE approval in there. Maybe instead of "team has decided what solutions" it's "OCTO-DE has approved the solutions..." Thank you!
