Define required data/information to initiate a non-visual prototype request

Issue Description

As a development team, I want to have access to non-visual prototypes for products and features we're building so that we can conduct research with people who use assistive technologies, (e.g. - Keyboard, Screen Reader) in order to make our research more inclusive.

This ticket is meant to define the data and information a team will need to provide when requesting to have a non-visual prototype built.

Here's what I have so far. I'm thinking all except the last will be required. form questions: Team name Lead researcher When do you need this? Link to sketch of prototype. Has the lead researcher signed off on this sketch? What research questions do you hope to answer? Is there anything else we need to know?
