Leaving this Medallia a11y issue here per a recent VA 508 Office audit of a newly digitized form Lay Witness Statement (VA Form 21-10210):

On the Feedback Survey dialog screen (Feedback), when the arrow keys are used to move the focus to the second set of radio buttons, “group start What is your overall satisfaction with this site? Not at all satisfied radio button not checked” is read—the state is read as part of the name. When using JAWS and navigating with tab, “Not at all satisfied radio button not checked 1 of 5” is read.
The Medallia team provided this update. Once staging is cleared (Debt Portal is launched). We will work with Medallia Resources and Your team to release the Medallia Version 2 design to reassess our surveys in 508
