Remove the follow up page to reduce overall form length

[ ] Page is removed from form config. Verified user flow is still working as expected after removal
[ ] Remove any unit and e2e tests associated with the page
[ ] Handle page migration (if user had a save in progress form and left off on this page, redirect to /file-disability-claim-form-21-526ez/new-disabilities/add Need to confirm)
