Issue Description

The online version of the 674 form is currently out-of-date with the paper form. SMEs shared that the new form had fields removed to make it easier to fill out.

Initial mapping of pdf changes is here.

Tasks

[x] Identify what fields need to be removed from the online form.
[x] Identify what fields need to be added to the online form (if any).
[x] Identify any language updates.
[x] Complete Design Intent meeting with the Platform Team
[x] Complete CAIA content review of designs
[x] Complete Midpoint Review with the Platform Team
[x] Address all Platform comments

Acceptance Criteria

[x] Figma designs of changes to 674 form ready for handoff to engineering.

Hoping to sign off on content early this week pending Kayce White's approval and setting up alignment of H1 with the label in CST.

Regarding questions 9A-B, I discovered we ask for the name of the program/school on a subsequent page. I asked for clarity in the CAIA ticket on which PDF question the program/school date is equivalent to a well as whether we're good to include an "other" option in the program/benefit checkboxes.

Moved all pages except "Add student" and "Review" to "Ready for Dev."
Per CAIA recommendations, I added a "Another program" option to the programs checkbox group in 674 and removed the "When did the student enroll in the benefit or program" question because it was redundant with a previous questions we're already mapping to 9C in the PDF.
Just waiting to see if we need to keep the student financial info section

@steele-lm I removed the "Has your student received income in the last 365 days" and moved the 674 step to ready for dev since we confirmed we need to keep the more complex financial info for now.
