Audit Forms 21-4142 and 21-4142a vs. VA.gov to identify changes for sync

Issue Description

The 526ez form gets updated when necessary. This epic covers the changes required on the digital 526ez form on va.gov to make it sync with the paper version.

The scope of this ticket includes identifying discrepancies between the VA.gov and current paper versions of:

• Form 21-4142, Authorization to Disclose Information to the Department of Veterans Affairs (VA)
• Form 21-4142a, General Release for Medical Provider Information to the Department of Veterans Affairs (VA)

This includes but is not limited to:

• Content changes where the paper ancillary form content has been updated
• Design/flow changes where the paper form change needs a design or flow change
• Possible use of existing digitized forms

Questions that arose out of this audit:

• Are upload screens (eg. Veteran uploads their own medical records) considered part of the digital ancillary 4142?
• What are the lay statement screens considered? There is the VA Form 21-10210 that is downloadable within the the online form.
• How is document management/release handled for BDD? Is it with the 526 or through ancillary forms? BDD requires two additional forms in Step 3: Service Treatment Records and the Separation Health Assessment.
