Finding from 526 Paper Form Sync Audit: Row 03 (Sharepoint link)

Column 4 "Approximate date disabilities began or worsened" is missing from the digital form. Scope - The scope of this ticket includes discovery and design for adding column 4 to the form.

Tasks

• [ ] Evaluate and propose placement of new question (note: we currently ask for "when this happened" on the fourth radio button's more info text fields)
