[UX] 10-10CG June Form Updates - Stakeholder Content Check Docs #83393

Issue Description

The CG Stakeholders all requested access to the Figma designs/wireframes during the weekly sync call on 5/17/21. this work is to export and package up all screen updates and annotations after CAIA review and the VADS office hours meeting.

Hello -

Please find attached a PDF document of the updated CG online form questions and screens. This is after completing the full review of the form with the VA.gov sitewide content team to ensure everything is current with the the latest patterns and standards online. The document shows a side-by-side of what is in production and what is updated along with some short highlights calling out what was changed.

Two items are flagged for now needing more clarification. We can discuss in the next meeting and welcome emailed clarification too! On the latest paper form under the representative definition there is a mention of some VA forms: VA Form 21-0972, VA Form 21-22 or VA Form 21-22a. Is the expectation that these have to be completed prior to completing the caregiver application or that they can be submitted with the online application via upload and the staff will get them routed? These forms links have been added to the drop-down example list on the representative page but we want to be clear in the explanation if they have to already have submitted one of these forms and are uploading a copy of that approval or how that process works. The county field. That isn't in alignment with address collection standards and we are looking into what other options are available to help with that user experience. Can you all please explain more about why it is collected for all applicants vs just the Veteran and what the impact to the applicants is?

Other than those clarifying questions, let us know if there is anything incorrect with these updates.
