10-7959c: update copy - Health insurance section (Primary)

As a user on VA.gov, I want to fill out the OHI form with the latest information so that I can report any changes to my other health insurance.

Page elements:

• primary health insurance (answer options: yes,no)
  • if no, then user skips to signature screen
• primary health insurance information (name, start date, termination date - not required)
• insurance type (answer options: yes, no)

Acceptance criteria:

• Given the user is on VA.gov and is filling out the OHI form
• When the user gets to the health insurance information
• Then the user can include that information as part of the other health insurance certification and it is displayed on the PDF attachment

• If user selects "no" to health insurance, they proceed to signature screen
• Insurance info: Remove "For example: January 19 2000" above both date entry fields (https://staging.va.gov/health-care/champva/other-insurance-form-10-7959c/insurance-info)
• Insurance plan screen: the radio for PrescriptionDiscount needs a space and "program": Prescription Discount program
• schedule of benefits upload page (can the dropdown read "schedule of benefits document" rather than "card")?

• [X] Investigate text wrap on insurance plan radio options -> The VA radio component has a 320px width cap on radio option labels, which is where the wrap comes from.

• Update header to "...health insurance card" (singular and remove "Required":https://staging.va.gov/health-care/champva/other-insurance-form-10-7959c/insurance-upload
• An error appears on the termination date fields if you are using your keyboard to tab through the form
