Acceptance Criteria

[x] Obtain screenshot for privacy officer by testing this in staging
[x] VA Privacy Officer contacted for approval
[x] VA Privacy Officer Approval
[x] Production template pushed to VA Notify
[x] Add form4142_upload_failure_notification_template_id to production manifests directory
[x] Create production mailer monitor in DataDog in existing production dashboard (link)
[x] Get all clear from research that this is included in the product guide
[ ] Contact Center notified and replied good to go for release
[ ] Turn on form526_send_4142_failure_notification flipper in production
