Move supply-reorder app into MHV portal

User story

As a Veteran, I want the supply-reordering form to be part of the MHV portal experience, so that I build a sense that every task related to my health care is in the portal and I am less likely to leave the portal on accident in the future.

• Created this epic to pull together a lot of tickets that emerged as we got into the work.

Possible tasks:

Acceptance criteria

• Supply reorder app exists under the /my-health/ url
• Entry paths to the app ensure that users with account gaps (LOA1, not-registered) have no worse an experience with the app than they did prior to its move
• Entry paths to the app ensure that the user is authenticated before they arrive at the /introduction page
  • Is this strictly necessary? Current state, I get a 404 when trying to reach the form at its new destination, https://staging.va.gov/my-health/order-hearing-aid-or-CPAP-supplies-form/introduction. However, what would happen if that url exists but the user is not signed in? E.g., the behavior when trying to reach the SM url without being signed in already is a login flow begins with ?next=%2Fmy-health%2Fsecure-messages to get the user to their intended destination after signing in.
• User feedback and analytics monitored for any signs that the UX was impaired by the move
• Tech debt and dead code is cleaned up

June 2024 update: this hit some complexity with signin widgets, then collided with a stakeholder-requested consolidation of the 2-3 entry-point pages in the benefit hubs. That consolidated page launched at the end of June. Patrick requested that we let that settle before proceeding with moving the app.

Decision to make/confirm: whether we add "verify" functionality to the sign-in widget before we move the app. It sounds like that would be CAIA's preference. Epic #85934.
