10-7959a: Beneficiary information

As a VA.gov user, I want to fill out the CHAMPVA claim form with the beneficiary information so that I can file a claim.

  • name/dob
  • identification info
  • 3rd party signer:
  • address selection
  • mailing address
  • phone number
• Language: "Your" if user is beneficiary, else use beneficiary's first name in brackets

Acceptance criteria:

• Given the user is on VA.gov and about to fill out the CHAMPVA claim form
• When the user gets to the beneficiary info page
• Then the user can fill out the beneficiary's information

@michaelclement here are a few changes:

You are the beneficiary:

• beneficiary mailing address: missing "We'll send any important..." sentence at top
  • bottom: lower case "your" mailing address, remove "has", add period at the end of "address"
• contact info: change header to "phone number" (from "contact information")
  • change "more" to "any" questions

Beneficiary is not you:

• beneficiary address: change "Does [the beneficiary]..." to "Does [beneficiary name]"
• mailing address: missing "We'll send any important..." sentence at top
  • bottom: remove last name, add period after 'new address'
• contact info: remove 's (when it's the "beneficiary name) and change to "any questions"

• I didn't catch it earlier but we should change on the identification screen as well, 'the beneficiary' to 'your' in the first sentence (when you are the beneficiary): https://staging.va.gov/family-and-caregiver-benefits/health-and-disability/file-champva-claim-10-7959a/beneficiary-identification-info
• Information (https://staging.va.gov/family-and-caregiver-benefits/health-and-disability/file-champva-claim-10-7959a/beneficiary-info): change 'information' to 'name' in first sentence
