As a CHAMPVA applicant, I want unnecessary fields removed from form 10-10d.

Acceptance criteria

The description and field for VA file number no longer appears under SSN on the Sponsor’s identification information page.
The description and field for VA file number no longer appears under SSN on the (Applicants)’s identification information page.
The VA file number field is hidden anywhere else it might appear (review page, etc)

Done in this GH branch, we can move forward on this after go-live.
