After several discussions with our team, stakeholders, Nancy Clarkson, and Matt Dingee, we have received approval to retain the demographic questions. We have collaborated with CAIA on both the disclaimer language and the placement of these questions within the form, ensuring alignment with their feedback. You will find these questions, along with the disclaimer ("stop") page, at the end of step 1 ("Your personal information").
The application will be unauthenticated, meaning no fields will be pre-filled. The email will only be asked for once, during step 1.
For the "Relationship to Veteran" section, it is being used in the context of "which one best describes you?" based on the form’s flow.
Upon further stakeholder feedback, we have been asked to incorporate a couple of additional options for the demographics question. We will ensure that these changes align with the format previously advised by Matt Dingee.
