Issue Description

Review low-volume error log issues for all forms and identify any that could be associated with production issues.

Background: Steve suggested this approach based on prod issues discovered by the Decision Review team after conducting a similar audit.

Tasks

[ ] Review logs for low-volume errors for: burial form

Acceptance Criteria

[ ] Possible production issues identified and documented
[ ] Follow on tickets for additional research or root cause fixes created

@sanjabaj2 will confirm what the ask is here. Is the goal here to capture low volume periods for the application?

@mjknight50 Steve suggested that all teams audit their logs for low volume errors to see if any of them can be fixed. The Decision Review team recently did this and uncovered some front end validation errors.
