Representatives may have other names they use that they want to be publicly visible.

[ ] Expand capacity in the “names” section and allow users to add a preferred name and any previous names used.
[ ] Add a middle name and suffix field to align with 21a.
[ ] Require selection of a primary name to display on Find a Rep
