OH - Add support for request flow

Description

As part of the Oracle Health request effort:

• When va_online_scheduling_OH_request feature toggle is turned on & User selects Food and Nutrition as type of care, allow patient to request an appointment at a Cerner site.
• Include check for va_online_scheduling_OH_request flag in FacilitiesRadioWidget: src/applications/vaos/new-appointment/components/VAFacilityPage/FacilitiesRadioWidget.jsx
• Include check for va_online_scheduling_OH_request flag in newAppointmentFlow src/applications/vaos/new-appointment/newAppointmentFlow.js

Moving this ticket to External Dependencies. Trying to validate the request flow for Judy in staging for Nutrition and food.

Need to see how we can get facility ID 668 configured in staging. (Drew/Apothesource is trying to see if they can set it up).

Unable to validate in the current flow in the application since the facility configuration is not set up for request or direct scheduling for Grandstaff.

Also to note- OH staging is down until 11/19. Facility configuration for OH is handled through the services (according to Brad). We were given the thumbs up that we can validate in staging the request and direct schedule flow with VPG service.
