**Note:** Code numbers changed during this time period. We'll accept any relevant evidence showing that you had regular contact with C-123 aircraft.

Notes:
I streamlined some of the structure and categorization since I don't think they will be useful to the Veteran. I don't think they will care or think about whether they were active duty or reserve, or whether they were stationed in the US or abroad. I think they will simply look for their unit number, which is most easily divided by airport.
I also removed what I think are irrelevant distinctions, like the "ferry flights" tag. Based on my reading about how agent orange claims are handled, I don't think this matters and only serves to confuse or discourage. But the PDF is pretty vague and the SME may countermand this.
I did my best with the unit/date formatting for the locations, but it's an awkward thing to render. I'm open to any alternatives that might occur to you.
