Update My HealtheVet content in platform documentation

Description

The Identity team is updating the platform developer documentation for all of our Credential Service Providers (CSPs) to ensure it's consistent and accurate. I've drafted some updates and additions to the documentation in a Google document that's been reviewed by the Identity Platform team.

Note: these updates are only related to My HealtheVet as a CSP. Some sections were not edited because they're owned by separate teams. I've been in touch with those teams and they confirmed they'll submit their own edits separately.
