Amend Appoint MVP user flow

Background

There have been some inconsistencies with the user flow in Staging:

• [x] claimant-personal-information routes to claimant-contact-phone-email, when it should route to claimant-contact-mailing
• [x] claimant-contact-mailing routes to veteran-contact-phone-email-for-non-veteran, when it should route to claimant-contact-phone-email
• [x] veteran-contact-phone-email-for-non-veteran should be removed from the user flow
• [x] veteran-service is shown incorrectly in the 21-22 flow -- this question is only for the 21-22a form
• [x] authorize-medical should route to authorize-medical/select IF the "Yes, but they can only access some..." radio option is selected
• [x] authorize-inside-va, authorize-outside-va, and authorize-outside-va/names are not available in the 21-22a flow

These should be addressed so that the experience in Staging matches our intended user flow (documented in Appoint MVP Planning Mural).

• [x] The Appoint MVP experience in Staging matches the intended user flow.

Some of the screens may be relying on a legacy rep type radio selection. Just need to get screens off of that reference.
