10-7959c Add VA notify template to send ZSF emails #97009

As the IVC forms team, I need a VA notify template so that I can send ZSF notifications to users who submitted a form that resulted in a silent failure.

Acceptance criteria

Our batch process has a template to call when we need to send a ZSF notification specific to 10-7959c
IVC forms team can manage the content
We have variables we can use in the email content for First Name, Form Name, and date the form was submitted
