API-31670 Add Legacy Appeals API v0 test account page #433

This page is equivalent to the page in #429, but it is for the Legacy Appeals API v0 instead of the Appeals Status API. This is nearly identical to #432, since both APIs have a single endpoint with the same required single parameter.

This PR will stay in Draft mode until the API is live.

This PR has been updated to take into account the recent changes made by Pivot! to the Benefits Test Accounts page. See latest commit on this PR for the detailed changes.
